this can also be done via online using your My.SSS acct
How do SSS compute for the TOTAL amount of Maternity benefit?
They based the computation according to the AMOUNT and NUMBER of your contributions made prior to conception. Usually its 9 months before you conceive.
"9 months bago ka nabuntis, ay nakapagbayad ka ng SSS contributions."
When can I receive the Maternity Loan?
I received mine 3 months after I submitted the MAT2 Form.
This is in the form of CHECK.
What is MAT2?
Maternity Form 2 is the "Maternity Reimbursement Application" Form
Your doctor or hospital will accomplish the one to accomplish it after your delivery (normal or ceasarian) stillbirth, or miscarriage.
You mean I can still receive the Maternity Loan even if i had CEASARIAN, STILLBIRTH or MISCARRIAGE?
YES. provided you accomplished all the requirements and submitted it to your SSS Office.

At the MAIN SSS BRANCH

I am 8months pregnant due on 12August2012. Currently I am on probation, the company already submitted my MAT-1, but for various reasons i decided to not accept the regularization a month before i reached 6month which will be on 23July2012. And i am planning to process instead my Maternity benefits as voluntary. My question is The MAT-1 that the company submitted on my behalf can this be carry over as voluntary so i dont have to submit another MAT-1. I am worried the SSS would deny my Maternity Benefits because of this sudden change from employed to voluntary.

ReplyDeletetanong ko lng po,paano po b ang gagawin ko kc po mahigit isang bwan na po simula ng maverify ko yung reinbursement ko sa maternity ko pero hindi parin po dumarating yung cheke ko,pinuntahan ko sa main ofis para itanong ang sabi po naihulog na po nila sa post ofis d2 sa caloocan yung cheke ng jan.15,binigyan po aq ng papel na ipapakita ko sa post ofis nmen na may nakalagay na check#, pero nung pinunthan ko po sa caloocan post ofis hindi p daw po nakakarating sa knila mahigit isang bwan na po yun,paano po kaya ang gagawin ko ??

DeleteIf nairelease na po ito ng SSS, Ibig sabihin po nasa safekeeping na po ito ng COURIER. Pls call SSS Hotline or go to the office who issued the check to Get the CHEQUE NUMBER and some details like when it was sent? Is the address Correct?
Tignan mo po ang LOGBOOK ng CALOOCAN POST OFFICE. Para ma -trace mo kung sinong DELIVERY MAN ang in charge sa delivery ng cheque mo... Pwede ka mag-request ng action na ito sa Mismong OIC ng POst office.

Deleteif alam mo na ang CHEQUE NUMBER, pls inform the SSS Main Office na HINDI MO PA NARERECEIVE ang Cheque.
So, they can make a STOP PAYMENT ORDER for that cheque. Ibig sabihin, ipapa-stop po nila ang pag eencash at issuehan ka nila ng NEW Cheque...
By the way, Yung Maternity cheque ko po was delivered almost 1 and half months... Pde ka rin maghintay ng konting araw pa...

na deny po ung maternity claim ko due to abrupt contributions hinahanapan po ako ng rs5 which i have when i first pay my contributions as ofw but then nung nag pa adjust ako ng pyment pinasign lng saken ung paper na declaration of contribution from sss then ngbayad nko at pasok nman sa system ng sss.now denied ung claim ko ano dapat gawin don para maging ok ung claim ko since inallow nman ako ng sss na iadjust at nireceive pyment ko bkit po ngyn p ngkaroon ng problem please advice me what to do po thanks..
ReplyDeleteask q lang kasi nagsubmit n aq ng smec bank which means s bank ipapasok ung maternity benefits then now i check the account through sss website,..which is my maternity claim is denied?then remarks it " Your Employer not enrolled to SMB"?
ReplyDeletethen why?all my requirements is done...lahat nasubmit n ng employer..nag enroll narin s SMB s bpi tapos nireceive n ng sss ok n daw wait lang ng one month.eh 1 month n ito tas sasabihin d pa daw enrolled?? kalokohan
ask ko lang pwde ba ko kumuha ng single savings sa kahit anong branch ng BPI
